No. Nothing happens to a file until you pick a time for it. Ignore the prompt and the file simply stays; after a grace period you set (5 minutes by default) TempCat applies your default choice, which is "Keep it" unless you change it. Files that were already in Downloads before you installed TempCat are never touched.
Rename it, or move it into a subfolder of Downloads, and it is still trashed on time. Move it anywhere outside Downloads and TempCat lets go of it: it counts as kept.
By default files go to the Trash, so you can pull one back until you empty it. If you prefer, Settings has a permanent-delete option.
